Before you register

Before starting your registration, be sure that you're eligible to register.

You're eligible if: you (or the person you are registering on behalf of) is registered or registrable as sight impaired (SI) or severely sight impaired (SSI), aged 29 or under and live within Sussex.

Not eligible? Don't worry! We may still be able to help you by signposting you to other services and support. Please contact us.

The process of registering online may take up to ten minutes. Once complete, a member of our team will be in touch to welcome you and provide any further information or support. If you require additional support to complete the registration, please contact us so that we can assist you.

Please note, if you are registering on behalf of somebody else, you must obtain permission from them prior to registering.

About you

Making a referral?

If you're registering on behalf of somebody else, it's important that we know a little bit about you (the referrer) too as we may need to contact you to obtain more information about the applicant.

Your contact details

Please be sure to provide accurate contact information so that we can get in touch with you. If you are registering on behalf of somebody else, please include their contact details.

This is the name we will use when contacting you. By default this is your forename and surname that you provided above. If you are a parent/caregiver of the applicant, in some cases it is more helpful to provide your name.

Please indicate your relation to the applicant e.g. Mother, Father. If you are registering on behalf of yourself, you do not need to indicate a relation.

How would you prefer to be contacted by us? By not indicating your preference, it is assumed you are happy to be contacted via any of the below methods.

Your sight impairment

It's important that you tell us about your sight impairment so that we know how best to support you. We appreciate that you may not be able to answer all of these questions, but please try to provide as much information as you can.

It's helpful for us to understand as much as we can about how your sight impairment affects you so that we know how best to support your specific needs. It can also be helpful for us to know about any other needs you may have.

Your education

We are dedicated to promoting education throughout our range of services, so it's helpful for us to know about your school/college/university so we can offer appropriate support for you.

This could be a school, college or university.

A QTVI is a Qualified Teacher of Children and Young People with Vision Impairment that offers support to students as part of your local authorities educational support services.

A SENCO is a Special Educational Needs Coordinator within a school who is responsible for assessing, planning and monitoring the progress of children with special educational needs and disabilities (SEND).

Please provide any additional information that you feel may be helpful for us to be aware of in relation to your education.
